Practice Head and Neck Ultrasound Guided Biopsies

A transparent gelatin mold (ultrasound phantom) is highly recommended as a training tool while learning how to perform ultrasound guided biopsies. It helps understand the 3 dimensional ultrasound image and hand-eye coordination needed for this sonography procedure.

How To Make Your Own Head and Neck Ultrasound Guided Biopsy Phantom in 9 Steps

Necessities:

1 container (no deeper than 2 inches)* 8 packets of Knox gelatin 5 olives filled with pimento 4 plastic forks 2 disposable cups Hot water Plastic wrap

Pour 2 gelatin packets into container;
Spread the content over the bottom. Slowly add hot water to gelatin;
Dissolve gelatin while stirring with a fork;
Make sure you form as few bubbles as possible. After the gelatin is dissolved fill container halfway;
To speed up the process use cool water;
Add an ice cube or two for even speedier results;
Place in fridge and let solidify. Place olives in the center of the solidified layer. Place one packet of Knox gelatin in a clean cup;
Dissolve gelatin with hot water and a fork;
Add some cold water and one ice cube to cool mixture;
Use no more than 8 oz. of fluid;
Make sure there are no bubbles in this layer that will interfere and create artifacts in the Ultrasound image. Place semi coagulated mixture (room temperature) directly over olives;
Place in fridge to further harden. Make more gelatin following Step Five;
Use 4 packets making one at a time;
The batch needs to be enough to nearly fill the container the container;
Make sure the liquid will not melt the lower layer. Make one more packet of gelatin;
Add while container rests in fridge to prevent overflowing. Put a few drops of water on the hardened surface;
Lightly spread water over surface;
Place plastic wrap over mold and smooth out wrinkles;
Plastic protects the surface somewhat from cracking.

*TakeAlong container by Rubbermaid works best for two to three participants

How To Use The Head and Neck Ultrasound Guided Biopsy Phantom You Made

Ultrasound probe placement should be in the non-dominant hand.

A light touch is needed during ultrasound guided biopsies - the gelatin mold facilitates this since it will crack if too much pressure is applied.

Move the ultrasound probe/transducer over the phantom and understand the placement of the olives under the probe and on the screen.

Decide where the needle should be placed under the probe. Watch the top of the screen to see where the needle appears in the screen. Note if the needle is traveling towards the "nodule".

Learn to appreciate the fact that: it is not possible to follow the needle if the transducer is not moved when using the "perpendicular" biopsy approach the needle will appear as a smaller than expected object on the screen very small controlled movements of the probe are best.

Tracks created by the needle placement will remain visible in the gelatin. Move the container around to get another spot to retry a biopsy.

Having a Digital SLR camera to use is the ultimate thrill for a budding photographer. Taking great sunset shots should be easy with such a tool, surely? You would think so, yet I know people who have invested in decent SLR's - but still could not create stunning sunset shots that even their previous compact camera could take. This digital photography for beginners article therefore suggests steps you can take to optimise your digital SLR for sunset photographs.

Many compact cameras have a setting for sunsets, yet DSLR's don't, so you have to take control of the situation. Turn off your fully auto, or basic shooting modes, and use your creative zone modes of P (program mode), Tv (shutter priority), Av (aperture priority) or M (manual) so that you have full control over the camera's parameters.

Auto White Balance can neutralise a sunset's glow, so switch to Manual White Balance and choose a setting of Cloudy or Shade.

Most DSLR's give you a number of "user defined" Picture Styles, so set one of these up for taking sunsets. Leave sharpness as standard, and add a little saturation.

You need to reduce the contrast to avoid your final image appearing over silhouetted. Adjust the Contrast slider in your user defined Picture Style one or two notches to the left. A reddish colour cast can also be achieved by doing the same with your Colour Tone settings.

Set your ISO at the lowest setting of ISO100. There's usually plenty of brightness in the sky, so this is fine. Sunsets can suffer from grainy images with higher ISO settings.

Exposure is important in any sunset shot. It's best to switch to Partial metering mode and take a manual meter reading. This can be a difficult aspect of digital photography for beginners to understand, so I'll explain how to do this. Once in Partial metering mode, point the camera at a bright area of the sky, directly above the setting sun. Press the Star button on the back of the camera to take a light reading and to set the exposure. Then compose your shot and press the shutter release to drive the autofocus and capture the image.

Consider your composition. Silhouetted objects and figures in the foreground will add interest. Reflective surfaces, such as lake water, the sea or wet roads will bring added colour to the shot.

Beware - once the sun starts to near the horizon, it sinks surprisingly quickly. This also happens to be the optimum moment when you will catch the strongest shots, yet it can come and go in an instant. Be prepared - make sure all your settings are in place well before this time.

When the sun has disappeared, it doesn't mean you should. For another thirty minutes or so, there can often be some amazing colour across the sky, so keep looking for more opportunities for great photos.

The word aperture means small hole or gap. In terms of photography the aperture is an adjustable opening used to control how much light gets in to the camera. Light enters the camera through the lens which uses a calibrated scale to determine the maximum and minimum aperture. This scale is written in f/numbers which are sometimes called stops. Sometimes beginners struggle to understand aperture fully. This is because a small aperture is denoted by a big number (such as f32) and a large aperture is denoted by a small number (such as f1.4).

Aperture is very important in macro photography as it is used to control the depth of field. The depth of field is the area in the picture that is in sharp focus. There is always going to be a small depth of field in macro photography due to the high magnification involved and the short working distance between the camera and the subject. Using a large aperture makes the depth of field smaller but will allow a faster shutter speed to be used. This reduces the chance of the picture being ruined by camera shake. Using a small aperture means using a slower shutter speed and increases the risk of camera shake. A sturdy tripod and a remote switch make it easier to get consistently sharp shots. Outdoor macro photography is even more of a challenge as you have the weather to contend with as well.

The main problem to overcome with macro photography is lighting. At high magnification and short working distance not much light is available. TTL flash is the only reasonable work-around. In-built camera flash is not very helpful in macro photography because it is set too far away from the subject. Special flash systems can be acquired which attach to the end of the camera lens. The most popular ones are called ring flash and usually contain one or two controllable fluorescent tubes. There are many other flash systems that you can use for macro photography. Twin flash is another popular system that contains two small flash units that attach to the camera lens. Some photographers attach a standard flash unit to an adjustable bracket or arm. This directs light directly above the subject. Flash allows the photographer to work with faster shutter speeds and smaller apertures and reduces the risks of camera shake. At high magnification the slightest movement of camera or subject will blur the image and ruin the shot. Flash photography can have its own problems, often making the images look unnatural. Sometimes too much light will reflect from the subject causing over exposure. To overcome this flash units have to be adjusted to provide the correct amount of light. This can be achieved by adding a special cover called a diffuser which lowers the power of the flash making it less harsh. There are several disadvantages of adding a flash unit. The added weight can make the camera more cumbersome and more difficult to hold. More time is required to set up the flash for each shot. If you are serious about macro photography a flash unit and a tripod are required to get consistently sharp images with good exposure. A remote switch is a small device that operates the camera from distance. Most macro photographers use a remote a switch to reduce camera shake. Remote switches are either remote controlled or have a short cable to the camera.

I hope this has been a useful introductory tutorial about the use of aperture in macro photography. Most of all put your camera into aperture priority mode and have some fun with macro. In aperture priority mode the camera will select the best shutter speed to match the aperture you have entered manually. This gives the photographer control over the depth of field and therefore control over the resulting image. Confident and credible images are the outcome of knowing how to adjust the aperture.
